Lerners is pleased to support The Transition Readiness Adolescent Complications Clinic (TRACC) at Children's Hospital, London Health Sciences Centre. The Clinic supports patient's ages 17-18 years with diabetes to prepare for transition from paediatric care to adult care through structured assessment, education and complications screening. Benefits of this program include the ability for health care providers to identify adolescents who need extra support through the transition process, encourage positive self-care behaviours, educate about potential life threatening complications, and will assist adolescents through navigating the adult system and follow-up care.
